The trust and confidence you have placed in us is most appreciated. Our mission is to help you to achieve your treatment goals, and to maintain optimal health over the long-term using safe, natural and holistic acupuncture, herbal therapy ( when appropriate), and nutritional guidance and support.
We will need you to fill out forms before treatment at your first visit. Please either arrive 15 minutes before your appointment time to complete these forms or print and complete them in advance so that you will have more time to talk to your acupuncturist during your consultation and not need to come early.
Remember to bring your completed forms with you to your first visit. We also ask that you bring any supplements or medications you are taking regularly, and copies of any relevant laboratory results or doctor's notes.

- Welcome letter: Please read the Welcome letter first, as it provides essential information regarding how our clinic functions. You may wish to print this out for you reference.
- Informed Consent: Please review the informed consent document as it provides important information about possible benefits and side effects of acupuncture and Traditional Chinese Medicine (TCM). Please sign this form and bring it with you to your appointment.
- New Patient Intake Form and Medical History: Please complete this form with as much specificity as possible. This will assist your physician in recommending the appropriate treatment strategy for your issue(s). Please bring this completed form with you to your appointment.
What to Expect at Your First Appointment
During your first visit, the Chinese Medical Physician will meet with you for a brief intake interview and physical assessment. During the intake, you will be asked many questions, some related specifically to you your complaint and others seemingly unrelated. Traditional Chinese Medicine (TCM) requires the entire person be taken into consideration so we can determine what is causing the condition or disease. We treat the whole person, no just the symptoms. Virtually every patient requires a course of treatment rather that a single treatment in order to get the positive results they want from TCM. One or all of the basic modalities of TCM may become part of your treatment plan and include acupuncture, bodywork, gua aha, cupping, and herbal medicine. At your first visit you will be advised about your treatment strategy. The initial session can last between 45 and 60 minutes. Follow up sessions may take less time.
